What is the GUIDE model?

The Guiding an Improved Dementia Experience (GUIDE) model is a Medicare program that pays healthcare providers for delivering key support services to people with dementia and their unpaid caregivers. The goal is to:

Improve quality of life for people living with dementia

Address behavioral and functional needs, coordinate care, & improve transitions between care settings.

Reduce burden and strain on unpaid caregivers

Provide caregiver training, referrals to community services, 24/7 support line, and respite services.

Prevent or delay long-term nursing home care:

Support caregivers to help people
with dementia remain safely at home.

Who can participate in GUIDE?

 All Medicare Part-B enrolled health care providers with the capabilities and expertise to provide ongoing, longitudinal care and support to people living with dementia. Participants include health systems, academic medical centers, physician group practices, community-based organizations, hospice and home health agencies, telehealth organizations, and other practices. 

GUIDE Participants may also partner with “Partner Organizations” (Medicare-enrolled or non-Medicare enrolled entities) to help them provide the care delivery requirements required under GUIDE.

GUIDE Participants can increase monthly payments per beneficiary (DCMP) by up to +10% for good performance on Performance-Based Adjustment (PBA) metrics, or lose as much as -3.5% for poor performance. Use of our platform drives improved performance on these metrics, which can be significant to your program’s bottom line.

on a population of just 200 beneficiaries is $125K.
